Pool Permission Form – MTW Summer & Team Adventure

This form is to give your child permission to accompany SHED to the Geisler State pool in Lawrence, MA on Monday, Tuesday, and Wednesday each week.
We will leave each day at 9:30 AM and return by 11:30 AM.
